About this template

Transition planning is legally required starting at age 16 (age 14 in some states), but many parents don't realize how critical it is until it's almost too late. This template helps you and your teen map out post-secondary education goals, employment preferences, independent living plans, and — crucially — which adult service agencies to connect with before your child ages out of the school system. Includes a checklist of agencies like Vocational Rehabilitation, DD Services, and SSI.
